They operate within local boroughs to provide a support service (No care) to vulnerable client groups which allows them to access the community and further support for a variety of needs around advice, guidance & signposting.
Your new role
Your role as a Home Service Coordinator is to support the manager in running the service effectively and efficiently for all those who are seeking support. This is largely done through the coordinating of the Support Workers to ensure the support required is fulfilled to the expectation of the client. You will also be responsible for managing referrals and providing assessments that create support plans that are then delivered. Additional tasks are to deal with day to day communication both internally & externally, keeping databases up to date & accurate, and ensuring adult safeguarding policies are followed at all times.
This role allows you to ensure those who most need support get it and, with expectations for the service to grow, it gives you the chance to lead from the front and put your stamp on the local area.
What you'll need to succeed
To be the successful candidate for this role, you will have prior experience of working within the Social Care sector, the ability to correctly manage the coordination/rota on a periodic basis, and you will be able to undertake service user assessments to be able to identify their needs.
You must also hold a full UK driving license and be willing to use your own car for travelling.
