Start date: ASAP

Full Time: Mon - Fri - 36/hrs

Education / Qualifications

Registered Nurse - degree level or equivalent

Experience & Abilities

Knowledge of National Framework for NHS Continuing healthcare and Nursing Funded Care (Oct 2018 revised) and associated legislation and guidance

3 years post registration experience

Experience of establishing & maintaining good working relationships

Experience of imparting unwelcome news to patients and their carers for example regarding their prognosis

Skills and Abilities

Problem solving skills

Excellent negotiating skills (to promote change & when dealing with resistance to change)

Ability to assess patients with complex needs

Understanding of implications of working across the NHS and the independent sector

Ability to coordinate case conferences/MDT meetings

Personal Qualities

Assertive & sensitive to other people's feelings and attitudes - empathic

Awareness of own development needs

Commitment to collaborative working

Need to be a successful team player & enjoys working as part of a team

You must have appropriate UK professional registration.
