Job Description

Service Support Worker required to work from Bedford (MK42 0YN) with travel to Luton & Dunstable Hospital and surrounding areas in the Beds area.

Full Driving Licence essential (UK - manual) as you will be required to drive a company vehicle

NO PERSONAL CARE OR MEDICATION ADMINISTRATION

An international charity based organisation that provides care and support to patients who have recently been discharged from hospital

Description

Work as part of the Health & Crisis Service

Be a friendly face to someone in the local area that may be struggling to do things for themselves due to age, illness or vulnerability

Provide practical and emotional support (telephone calls, welfare checks, shopping)

Provide reactive and flexible support to adults following a hospital stay to enable early discharge

Respond to referrals from Clinicians and Community Health professionals

Support service users in the weeks after hospital discharge and/or preventing readmission

Take service users to hospital appointments, pick up prescriptions and shopping

Profile

Friendly and patience

Experience is desired but not essential - full support provided

Happy to work as a team but also on own initiative

UK Manual driving licence and happy to drive a company vehicle

Monday to Friday 10am - 6pm
