Position: Band 6 Support Worker
Location: West London
Duration: 3 to 6 months (with potential extension)
Schedule: Monday to Friday (9:00 am - 5:00 pm)
Start Date: ASAP
Pay Rate: £30 p/h

As a Band 6 Support Worker, you ll play a vital role in empowering individuals with complex needs to live fulfilling lives. Your day-to-day responsibilities will include:

• Supporting individuals with physical, mental health, and social care needs, enabling them to achieve their goals.
• Collaborating with a multidisciplinary team to ensure seamless care and service delivery.
• Conducting assessments and creating tailored support plans that promote independence and well-being.
• Providing guidance on harm reduction, health promotion, and relapse prevention strategies.
• Acting as a key contact for service users transitioning between services, ensuring smooth handovers and continuity of care.
• Responding promptly to safeguarding concerns with professionalism and care.
• Maintaining clear, accurate records that meet organizational and professional standards.

• Qualifications:
  • Social work experience essential.
  • May consider a nurse with substantial support worker/social worker experience.
• Experience: A strong background in health, social care, or substance misuse services is desirable.
• Skills: Exceptional communication, teamwork, and problem-solving abilities.
