Hospital Social Worker - Hammersmith and Fulham

Are you a dedicated and experienced Social Worker looking for a rewarding role in a hospital setting? Hammersmith and Fulham are seeking a skilled professional to join the team at Charing Cross Hospital.

Role: Hospital Social Worker

Location: Charing Cross Hospital, Hammersmith and Fulham

Rate: £35.50 per hour (agency position)

Contract: 3 months+

Work Arrangement: 2 days a week in the office

Key Responsibilities:

Undertake assessments for patients in a hospital setting, ensuring timely and safe discharges.

Work closely with healthcare professionals, patients, and families to develop robust care and support plans.

Provide advice and support to patients regarding available services and community resources.

Ensure compliance with the Care Act 2014 and other relevant legislation in all aspects of social work practice.

Maintain accurate and timely case records.

Requirements:

Social Work England (SWE) registration.

Experience in hospital social work, discharge planning, or related roles.

Strong understanding of safeguarding, mental capacity assessments, and care planning.

Ability to work effectively within a multidisciplinary team.
