Band 6 Care Coordinator NYYS AMH HHR Perinatal Team (Northallerton, NHS)

An exciting opportunity has opened for a Band 6 Care Coordinator to join the NYYS AMH HHR Perinatal Team, working within the NHS in Northallerton. Key Details:

• Position: Band 6 Care Coordinator Perinatal Team
• Location: Northallerton (Hybrid role with parking available nearby)
• Working Hours: Monday to Friday, 09:00am 17:00pm (37.5 hours per week)
• Contract Length: 3 6 months (ongoing)
• Start Date: December 2024 January 2025
• Pay Rate: £25 £28 per hour

The successful candidate will play a pivotal role in supporting individuals and families during the perinatal period. You will be part of a multidisciplinary team known for its compassionate, person-centered approach and its commitment to delivering innovative mental health care. Requirements:

• Proven experience as a Care Coordinator.
• Expertise in mental health and the ability to work effectively within multidisciplinary teams.
• Qualification as a Nurse, Occupational Therapist (OT), or Social Worker (SW).
