Job Title: Reablement Enabler

Location: Islington

Hours Per Week: 30 Hours

Rate of Pay: 20 P/H (Umbrella)

Islington Council are currently searching for a Reablement Enable to join their team.

Job Duties:

Deliver personal care and support based on personalized support/Reablement plans, including personal care, domestic tasks, and exercise programs.
Focus on enablement and promoting independence to help individuals achieve rehabilitation or recovery outcomes.
Work closely with health and social care professionals involved in care planning and delivery.
Provide care and support in an enabling way to promote individuals' independence and well-being.

The Candidate Will Have:

Level 2 NVQ or QCF Level 3
Experience of working for Adult Social Care or an equivalent agency providing domiciliary personal care
