Social Worker - Assessment and Care Team

Role Overview:
As a Social Worker within the Adults Assessment and Care Team, the successful candidate will work collaboratively with other professionals to assess the needs of adults with a variety of complex care needs, including physical disabilities, learning disabilities, mental health issues, and older adults. The role involves carrying out assessments, creating care plans, and supporting service users through the provision of tailored interventions that promote independence and well-being.

Key Responsibilities:

• Conduct comprehensive assessments of individuals' social care needs.
• Develop, review, and implement care plans in line with policies and procedures.
• Safeguard vulnerable adults, ensuring appropriate protection plans are in place.
• Collaborate with health professionals, family members, and other agencies to deliver effective person-centred care.
• Maintain accurate and up-to-date case records using relevant systems.
• Contribute to the development of service improvements within the team.

Person Specification:
The ideal candidate will be a qualified, experienced social worker with:

• A Social Work degree (or equivalent) and SWE registration.
• At least 2 years of post-qualifying experience in adults' social work.
• A strong understanding of the Care Act 2014, safeguarding, and mental capacity legislation.
• Excellent communication and assessment skills.
• The ability to work effectively within a multidisciplinary team.
• A flexible and compassionate approach to working with vulnerable adults.
