Connect2socialwork are urgently looking for a Social Worker to join their great Hospital team with a Pay rate up to £38 an hour on a locum basis.

Practice advice on complex cases.

Making professional decisions on cases and recommendations to team manager about risk.

Triaging safeguarding referrals from emergency services and making decisions.

Responding to queries via webchat.

Triaging phone or online referrals, in line with strengths-based practice.

Following up enquiries to confirm queries have been resolved.

Being available at specific times for individuals, unless otherwise agreed.

Identifying risk and escalating safeguarding concerns as necessary.

Preparing clear and concise reports for individuals' files that can be clearly understood by colleagues.

Recording feedback (positive and negative), in line with the appropriate policy.

Maintaining excellent working relationships with colleagues at all levels.

Building positive and professional relationships with stakeholders including the people who use our service, relatives and providers.

Conducting carers assessments.

Conducting wellbeing assessments to identify individual's level of needs and support available.

Conducting mental capacity assessments (where appropriate for the client group).

Pro-actively seeking to maintain up-to-date knowledge and skills to the expected standard, including changes to policies and legislation.

SWE Registration

Eligible to work in the UK

Drive and have access to a car

Post-qualified experience
