Job Advertisement: Band 6 Community Psychiatric Nurse (CPN)
Location: Warwickshire Area
Team: Community Mental Health Team (CMHT)
Start Date: ASAP
Rate: £26 per hour

We are seeking a dedicated and experienced Band 6 Community Psychiatric Nurse (CPN) to join our dynamic Community Mental Health Team in Warwickshire. This is a fantastic opportunity to make a real difference in the lives of individuals experiencing mental health challenges.

• Position: Band 6 CPN
• Responsibilities:
  • Provide care coordination for a caseload of approximately 30 service users.
  • Deliver high-quality, person-centered care plans.
  • Conduct mental health assessments and risk evaluations.
  • Work collaboratively within a multidisciplinary team.
  • Liaise with service users, carers, and other healthcare professionals.
• Hours: Full-time, Monday to Friday, 9:00 AM to 5:00 PM.
  • Part-time hours may be considered for the right candidate.
  • One day of remote working per week may be negotiable.

• Qualifications: Registered Mental Health Nurse (RMN) with valid NMC registration.
• Experience: Proven experience in a care coordination role within a CMHT.
• Skills:
  • Excellent communication and organizational skills.
  • Ability to manage a complex caseload effectively.
• Transport: Must hold a valid UK driving license and have access to a car for community visits.
