Hours: Full-time (5 days per week, flexible across a 7-day period)

Service Care Solutions is seeking a dedicated and experienced Band 6 Mental Health Practitioner to join an NHS team in Cambridgeshire. This role involves providing essential mental health support to older adults experiencing a mental health crisis within the community.
Key Responsibilities:

• Visiting older adults in the community who are experiencing a mental health crisis.
• Managing cases effectively in a community setting, reducing the need for hospital admissions.
• Delivering high-quality, patient-centred care as part of a multidisciplinary team.
• Maintaining detailed documentation and working within NHS guidelines.
