Social Prescribing Advisor
£28,997 FTE
12-month contract (Maternity Cover)
30-37 hours per week
Voluntary Action Rotherham is the lead body for supporting, developing and promoting the voluntary and community sector in the Rotherham borough. Our core activities and associated projects are a keystone for local community engagement and development. We are looking to recruit a Social Prescribing Advisor as part of our award-winning service.
We work with GP Practices, Mental Health teams and Hospitals to support individuals to tackle barriers that prevent them from self-managing their health and wellbeing and becoming connected to their local community.
Social prescribing is a mechanism for linking patients and those accessing care with local, non-medical sources of support, recognising that many things that affect our health like loneliness, stress, anxiety or low levels of activity can’t be treated by doctors or medicine alone. Social prescribing connects people to non-medical support to address these issues and other unmet needs.
Voluntary Action Rotherham hosts a range of Social Prescribing teams, and we currently have a 12-month fixed term role available for a Social Prescribing Advisor.
As a Social Prescribing Advisor, you will work alongside a range of local partners from the NHS, Adult Social Care and the Voluntary Sector. As a key partner in the delivery of multi-disciplinary support for patients, you will work with referred individuals to determine what matters to them most and co-design a personalised support plan which connects them to relevant support and empowers them to take control of their health and wellbeing, live independently and improve.
The successful candidate will need to:
Be enthusiastic and able to motivate and influence others
Be able to build positive relationships and work in partnership with a range of organisations and individuals
Have experience of working with Children, Young People and Families, particularly with individuals living in areas of deprivation and health inequality
Have an understanding of issues around mild to moderate mental health needs in Children and Young People
Be resilient, creative and able to respond positively to change
Have an understanding of the impact that social, economic and environmental factors have on health and wellbeing
Have excellent interpersonal skills
Work effectively as part of a team
Be able to write effective case notes and use data monitoring systems effectively
Understand the need for robust Information Governance including Data Protection and Confidentiality
Some evening and weekend work will be required to fulfil the nature of the role. Due to the nature of the role, the successful applicant will be subject to an Enhanced Disclosure and Barring Service check.
