Ardent Social is proud to partner with West Sussex County Council to recruit an experienced Social Worker for their Older People s Mental Health (OPMH) Team. This role offers an excellent opportunity to make a meaningful difference in the lives of people over 65 with complex mental health needs, including dementia, while working in a collaborative and supportive environment.

Key Responsibilities:

• Conduct Care Act assessments and S117 reviews for individuals with complex social care needs and mental health conditions.
• Undertake strengths-based assessments, applying relevant legislation, including the Mental Capacity Act, to empower individuals and promote autonomy.
• Develop and implement support plans aimed at improving wellbeing and achieving personalised outcomes.
• Manage a varied caseload involving complex cases, including risk management, safeguarding, and best interest decisions.
• Collaborate with multi-disciplinary teams and mental health professionals to deliver seamless care.
• Participate in joint work with health partners, including attending Court of Protection hearings and handling DOLS challenges.

What We re Looking For:

• Qualified Social Worker with current Social Work England registration.
• Experience in completing Care Act assessments and S117 reviews.
• Strong knowledge of the Mental Capacity Act, safeguarding, and related legislation.
• Ability to work across West Sussex, with a hybrid schedule of 2-3 days in the office (Chestnuts Day Centre, Bognor).
• A collaborative mindset with excellent communication and organisational skills.

Contract Details:

• Duration: Ongoing, with regular extension reviews.
• Location: Office based at Chestnuts Day Centre, Bognor, with travel across West Sussex (postcode: PO22 9JQ).
• Pay Rate: £35.00 - £38.00 per hour (umbrella).
