Home Health Occupational Therapist (Days) - San Ramon, CA Location: AccentCare San Ramon - 3170 Crow Canyon Pl Suite 270, San Ramon, CA 94583 Contract Length: 13 weeks Hours: 40 hours per week (flexibility required) Mileage Reimbursement: $0.67 per mile Job Overview: AccentCare San Ramon is seeking an experienced Occupational Therapist (OT) for a Home Health position. The ideal candidate must have true home health experience with OASIS documentation proficiency. OASIS-E experience after January 2023 is required. Key Responsibilities: Provide home-based occupational therapy services to patients across Alameda County Manage a caseload of 20-25 patients per week, averaging 5-7 productivity points per day Complete all documentation same day using Homecare Homebase (HCHB) Attend mandatory in-person office meetings once a week (Tues, Wed, or Thurs) Maintain a flexible schedule, with potential patient visits before or after standard office hours (8 AM - 5 PM) Requirements: License: Active California OT License Certification: BLS (Basic Life Support) Experience: Minimum 2 years in home health with OASIS-E documentation Technology: Proficiency in Homecare Homebase (HCHB) Work Gap: No more than 1 year gap in experience Additional Details: Floating: Must be flexible On-Call: No call requirements Orientation: 2-3 days (Modules + Team Introduction)
