Band 6 CHC Assessor – £40 per hour – Birmingham (Hybrid Working)

Location: Birmingham (Hybrid – must attend on-site visits)

Start Date: ASAP

Contract Length: 3 months (potential for ongoing contract)

Working Hours: Monday – Friday, 09:00 AM – 17:00 PM

Pay Rate: £40 per hour

We are seeking an experienced Band 6 CHC Assessor to join a dedicated team in Birmingham on a 3-month contract, with the potential for extension. This role offers flexible hybrid working, but the successful candidate must be available to conduct on-site patient assessments as required.

Key Responsibilities:

Conducting comprehensive Continuing Healthcare (CHC) assessments in line with national frameworks

Reviewing care packages and making recommendations for eligibility and funding

Liaising with multidisciplinary teams, patients, and families to ensure the best possible care

Ensuring compliance with NHS Continuing Healthcare guidelines and policies

Managing caseloads efficiently and maintaining accurate documentation

Requirements:

Registered Nurse, Social Worker, or Allied Health Professional with relevant CHC experience

Previous experience conducting CHC assessments and DST (Decision Support Tool) reviews

Strong understanding of NHS CHC framework and legislation

Excellent communication, analytical, and report-writing skills

Ability to work independently while collaborating with multidisciplinary teams
