Title: Registered Nurses / licensed practical nurse Company: State of Montana # of Openings: 1 Priority: High Position Type: Contract Location: Columbia, MT, United States Description: Long term care experience is required ESSENTIAL FUNCTIONS - RN • Assists in facilitating client care and collaborates with the Program and Operations Manager on the plan of care. • Observes, identifies, and documents any signs and symptoms of changes in the client's physical, emotional/mental/social condition, reactions and medications, and environmental safety factors. • Teaches, supervises, and counsels the client and/or caregivers in the home regarding the nursing care needs and other related problems of home health care to promote continuity of care. • Maintains accurate and timely records and observes the confidentiality of the medical record. • Participates in the review of recertification of physician's plan of care including the writing of nursing appraisals and discharge summaries as requested by the Program/Operations Manager. • Attends staff and interdisciplinary staff meetings, nurses' meetings, and in-services as scheduled. • Assists with ambulation and transfers. • Assists with range of motion exercises as per the plan of care. • Reports immediately to the nurse supervisor any changes in the client's condition. • May participate in the orientation of new nurses accompanying the RN on home visits. • Other duties as assigned. • CDCN reserves the right to change the job description without input from the employee.
