Hartlepool Borough Council is seeking a dedicated and passionate Children's Social Worker to join our Disabilities Team. We are committed to providing high-quality support and advocacy for children with disabilities and their families, empowering them to achieve their full potential.
About the Role:
You will be part of a team dedicated to:
Providing tailored support and interventions to children with disabilities and their families, addressing their needs and promoting their well-being.
Working collaboratively with children, their families, schools, health professionals, and other agencies to create individualised care plans.
Advocating for the needs of children with disabilities, ensuring their rights and access to appropriate services and resources.
Promoting inclusion and supporting children to participate fully in their communities.Responsibilities:
Conduct comprehensive assessments of children with disabilities and their families, identifying their needs, strengths, and aspirations.
Develop and implement individualised care plans, tailored to the specific needs of each child and family.
Provide direct support to children and families through one-to-one sessions, group work, and community outreach.
Facilitate access to specialist services and resources, such as education, healthcare, and social inclusion opportunities.
Work collaboratively with other professionals and agencies to ensure children receive the best possible care and support.
Monitor progress, evaluate interventions, and make adjustments as needed.
Advocate for the needs of children with disabilities, ensuring their rights and well-being are protected.
Maintain accurate records and documentation, adhering to relevant legislation and guidelines.
Participate in regular supervision, training, and professional development opportunities.Key Requirements:
Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
Social Care England registration.
Updated DBS check.
Experience working with children with disabilities and their families.
Knowledge of relevant legislation and guidance related to supporting children with disabilities.
Excellent communication, interpersonal, and advocacy skills.
Ability to build rapport with children, families, and other professionals.
