We are seeking a dedicated and compassionate Community Nurse to join our team in Devon. You will work closely with patients, families, and multidisciplinary teams to ensure excellent standards of care and support are provided.

Key Responsibilities:

• Conduct patient assessments, develop care plans, and administer treatments.
• Provide support and education to patients and their families regarding health conditions and treatments.
• Work collaboratively with other healthcare professionals to coordinate care.
• Monitor patient progress and adapt care plans as needed.
• Maintain accurate records and comply with healthcare regulations.

About You:

• Registered Nurse with a valid NMC pin.
• Previous experience in community nursing or a keen interest in working in the community.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• A compassionate and patient-focused approach.
