SEND Case Officer (Annual Reviews)

We are seeking a dedicated SEND Annual Reviews Officer with a focus on Phase Transfers to join a Local Authority. You will play a vital role in ensuring that Education, Health and Care Plans (EHCPs) are reviewed effectively and that transitions between key education phases (e.g., early years to primary, primary to secondary, secondary to post-16) are smooth, timely, and child centred.
Key Responsibilities:
Oversee and coordinate the annual review process for EHCPs, ensuring statutory deadlines are met.
Focus on phase transfers, ensuring that transition reviews are completed in a timely manner and that EHCPs are updated appropriately.
Work closely with families, schools, and other professionals to facilitate effective communication and decision-making.
Ensure compliance with SEND Code of Practice (2015) and relevant legislation.
Maintain accurate records and contribute to reports on service performance.
Provide advice and guidance to parents, carers, and educational settings regarding the annual review and transition process.
Essential:
Experience working within SEND services, education, or local authority settings.
Strong knowledge of the SEND Code of Practice and EHCP processes, particularly phase transfers.
Excellent organisational and time-management skills to meet statutory deadlines.
Ability to build positive relationships with families, schools, and other stakeholders.
Strong written and verbal communication skills.
Attention to detail and ability to manage sensitive and complex cases.
