About the Role:
We are seeking a dedicated and experienced Adult Social Worker to join our Assessment Team in North London. This is a full-time role offering a flexible hybrid working arrangement, combining office-based and remote work to support work-life balance. As an Adult Social Worker, you will play a vital role in assessing and supporting vulnerable adults to improve their quality of life and ensure they receive the appropriate services and care they require.
Key Responsibilities:
* Conducting assessments of adults who require social care services, including those with physical and/or mental health needs, disabilities, and older people.
* Developing person-centered care plans to meet individual needs and promoting independence and well-being.
* Coordinating services and interventions, working closely with health professionals, carers, and other agencies.
* Ensuring all assessments, care plans, and reviews are completed within set timescales and in line with statutory requirements.
* Managing a caseload effectively, ensuring all cases are handled with professionalism and compassion.
* Contributing to safeguarding efforts and conducting safeguarding investigations when required.
* Keeping accurate records and case notes in compliance with the relevant legal frameworks.
Requirements:
* Qualification: Social Work degree or equivalent.
* Experience: At least 2 years of post-qualification experience as a Social Worker, preferably within adult services or a similar setting.
* Skills: Excellent assessment and care planning skills; ability to work with individuals from diverse backgrounds and with complex needs.
* Knowledge: Sound understanding of relevant legislation such as the Care Act 2014, Safeguarding Adults, and the Mental Capacity Act.
* Registration: HCPC registered.
* Personality: Compassionate, empathetic, and a strong advocate for service users.
