What You'll Do:

* Conduct assessments and create personalized care plans for patients

* Visit patients in their homes, communities, or hospitals

* Collaborate with our care team to ensure the best outcomes for patients

* Provide ongoing support and supervise care packages

* Participate in reviews and multi-sign-offs (with extra compensation)

What We Need:

* A qualified Registered Nurse

* Strong assessment and care planning skills

* Excellent communication and teamwork abilities

* Flexibility to travel between locations

* Ability to work independently

We’ll provide training and ongoing support from our Clinical Manager and Director. Plus, competitive pay and extra compensation for multi-sign-off tasks!
