Service Care Solutions are recruiting for an Enhanced Frailty Nurse Practitioner to support elderly people aged 65 years and older, within the community. 

Enhanced Frailty Nurse Practitioner 

Pay: £35 LTD p.h 

Contract: Locum for 8 weeks 

Start: ASAP

Location: Harrow, Greater London 

Essential: 

- BSc Nursing or Emergency Care Practitioner Qualification

- Non-Medical Prescribing Qualification 

- Wound Care, including compression bandaging 

- Phlebotomy 

- Community working

Duties: 

- To reduce avoidable hospital admissions or re-admission or A&E attendances

- Provide clinical support to patients, supported by the wider Clinical team, to work autonomously holding their own caseload.

- Support the Enhanced Frailty Team in the delivery of Complex Care.

- Attend all mandatory meetings such as Touch Point meetings and MDTs.

- To maintain excellent working relationships with all those involved in the provision and development of services for those with long term conditions.
