Healthcare Assistant/Phlebotomist

We are looking for Healthcare Assistant for a community based role, supporting patients requiring enteral and parenteral nutrition. This is a full time role.Key Responsibilities:
Provide continual support to patients through a variety of channels including but not limited to home visits
To schedule own workload in conjunction with Nurse Team manager
To receive referrals from Nurse Team Manager and arrange contact with the patient to discuss the required training and subsequent visit(s)
To undertake reviews of patient’s requirements
To educate patients/carers on how to administer therapies, store the medication and who to contact if they have any questions
Accurately complete Nurse Visit Forms following each patient visit / telephone call
Inform the referring centres of any instances of non-compliance from the patient
To maintain up to date care plans and accurate clinical records
To always carry out your role in accordance with NMC Code of Practice and Bionical
To report any adverse drug events following the Bionical standard operating procedure
To attend any mandatory training as required, and to complete self-learning as required
To always practice good infection prevention practice by adhering to Infection Prevention and Control policies and procedures
To understand and be aware of your responsibilities relating to the safeguarding of all patients
To undertake other duties as may be required from time to time consistent with your grade and skillsPerson Specification:
The ability to put people at ease and communicate clearly through both verbal and written channels
A current driving licence and car is essential for this role
Experience in patient training
Attention to detail in all written documentation
Has the ability to build strong relationships with both colleagues, patients and clients
To have the flexibility to cope with the changing service demands
A high level of time management and organisational skillsQualifications & Experience
NVQ Level 2 Health and Social Care(desirable)
Experience in a community setting (desirable)
