Live Well Co-ordinator - Sandbach, Cheshire East

My client in Cheshire East is looking for a Live Well Co-ordinator to assist in achieving better outcomes for Service Users, Citizens, Adults, children, young people and their families through effective joint partnership working with staff, Providers, Partners, care professionals, and members of the public, services and stakeholders to support the effective delivery of integrated services relevant to Cheshire East Council's Information, Advice, and Marketplace Portal (Live Well).PRINCIPAL RESPONSIBILITIES1 To develop, and contribute to, the delivery of outstanding Personalised Online Services outcomes in relation to the Care Act and the Children and Families Act where relevant, within designated localities in line with National, Regional, County and Local Plans.2 Develop effective relationships with partners and support and respond to the work of staff, care professionals, and services operating within the locality to ensure that the best possible outcomes are secured for Citizens, particularly those at risk of poor outcomes.3 To work with all staff, Providers, Partners, to promote and strengthen partnership working and support the appropriate, collective commissioning of services for Service Users in the care of the Authority.4 Encourage staff, Providers, Partners to promote the full participation and engagement by Citizens including Adult and Children groups in the development and evaluation of a range of extended personalised services which will best meet their needs and provide best value for money.5 Identify ongoing revenue and access external funding streams to maximise the capacity, resources and sustainability of services to support the Live Well agenda and ensure that this is efficiently and appropriately deployed and accounted for in line with the established accounting arrangements, providing financial and other reports as required to an agreed cycle.6 Promote collaborative working and the sharing of good practice between Providers and partner agencies. Disseminate information, to an agreed timetable and in an agreed format7 Liaise with, and work alongside, other local authority officers and partners to monitor and evaluate the impact of activities around the Live Well agenda and in order to provide information, as required. Reshaping commissioning and extended services in response to outcomes. Provide reports, information and presentations as necessary to a range of stakeholders in line with local, county and national requirements.8 Provide appropriate support to ensure Live Well operates in line with legislation, policies and procedures set out nationally and by the Local Authority, including safeguarding, health and safety and equal opportunity.9 Undertake other duties when necessary commensurate with the levels of skill, knowledge and responsibilities of the post.The job holder may be required at any time to under take any work up to and/or at a level consistent with existing responsibilities.
