12 months Fixed Term contract
Working 35 hours per week, working flexibly between 9am and 6:30pm
Salary - £18,000 per annum
Our client provides a range of supported housing, hostel and floating support provision for homeless men and women, former rough sleepers and services that specialise in supporting vulnerable adults with complex needs. Services that are flexible and user-led, with a focus on prevention, enabling individuals to take control of their lives.
As Supported Housing Officer you will be responsible for providing a support service to vulnerable adults who are homeless by identifying their needs and agreeing a programme of support via floating support, telephone support or housing related support.
You will also be responsible for:
* Providing an intensive housing management and housing support service, whilst working in conjunction with housing providers.
* Giving practical and emotional support in order to assist, promote and enable service users to move on into independent accommodation.
* Being proactive in supporting and empowering service users i.e: accompanying them to external appointments and sometimes acting as their advocate e.g. GP’s, substance support services, health clinics etc.
* Carrying out risk assessments that lead to effective management plans that are monitored and reviewed regularly to ensure that risk is managed effectively and in line with our Risk Management Policy and procedures.
* Planning and delivering individual support packages that aim to meet the service users’ individual needs. Holding regular review meetings to highlight where outcomes have been met.
